* Cheap and Works ...
I bought this to power a video switcher that was missing the power supply. This was the cheapest replacement I could find. It came with many different sized adapters, plugged it in (watch polarity) and works fine.

OK if you are a hobbyist
As the previous reviewer suggested, the output is unregulated. That means it will "meet spec" as long as there's a load on it of 1,000 mA (hence the "required" wording on the info sheet). If not, the output will be a lot higher. In this case, about 17 V DC with no load (110 VAC input). As soon as you put a load on it, the output voltage drops somewhat close to the voltage you selected. The voltage selection is done by a switch located under the outlet prongs, behind a plastic cover. Not seen from the photo.
I used my adapter to power a DC fan, which doesn't care one way or the other about regulated voltages. I just measured the voltage under load and adjusted the voltage selection switch to get what I needed. In this case, I got 10.4 V with a 40 mA draw on the 6 V setting. It works.
If you're building something and looking for inexpensive way to get DC from the mains, this is it. If you need to power some expensive electronics, stay away.

* Possible Fire Hazard ...
I gave the product two stars only because it actually did supply power to my electronic device. However, the adapter gets very VERY hot when in use and I could also smell an electrical-burn type smell. I am worried that with extended use it might actually catch on fire. If you do use it I'd recommend never leaving it unattended, and don't place it near curtains or similar items. It is also huge and heavy, so it will block all other outlets in it's vicinity. Not very useful for portable devices such as DVD players you'd carry with you. I don't recommend purchasing this item.

UNREGULATED - Use Caution
This device is labeled as "Universal." Unlike those similar devices that cost 15 to 20 US dollars, this device is unregulated. I've seen a few other unregulated that costed 15 to 20 bucks, so this one is still a very good deal. Unregulated adapters only work safely with devices meeting the exact specs of the adapter. For instance, if you use a 300mA device with this adapter, the device and/or the adapter could fail, or worse, start a fire. So, at the very least, never use unattended. MODIFICATION - ATTEMPT AT YOUR OWN RISK: I am using these to power some cold cathode light kits (Logisys brand, 500mA - not verified). I use two kits per adapter. On one, I use three DC fans and one kit. No fires yet. You should know how to do basic wiring to do this mod. If you do any cutting, the white-striped sleeve goes to yellow or red. You can also solder the PC wiring to one of the adapter plugs, instead. That requires steady work to isolate each wire. Three stars due to the unregulated outputs.
